I understand that the activities associated with participating in gymnastics may lead to serious accidents causing bodily injury, disability and damage (including property). Ritmix Gymnastics club through its affiliation with Gymnastics Queensland Inc maintains limited insurance cover on members registered with Gymnastics Australia. I understand Ritmix Gymnastics Club will take all precautionary measures to ensure safety and injury prevention. I indemnify Ritmix Gymnastics Club and its staff against all liability arising as a result of my negligence (or the negligence of my servants or agents) in respect of any death, bodily injury, disability, damage (including property damage) occurring as a result of such negligence to the fullest extent permitted by law but only to the extent that Ritmix Gymnastics Club and its staff are not already indemnified or insured. I, being the guardian/parent of the above named athlete, acknowledge I have read this document and accept its terms on behalf of such athlete in order to participate in gymnastics activities administered by Ritmix Gymnastics Club.

Payment Policy

· By enrolling you are committing to payment of the entire term and are taking a class spot. · If you pay the term invoice late, a late fee of $25 will be charged to your account. Gymnasts are not permitted to participate unless the invoice is paid in full. New gymnasts starting part way through the term are charged on a pro-rata basis. · Your class booking is automatically carried over from term to term until you advise us otherwise. · Term fees are calculated on the number of school weeks in the term. Terms are calculated using State School term dates. · There are no refunds on class fee or registration fees, ‘change of mind’, and/or after the term commences under any circumstances. · Credit towards next term due to injury or medical reasons can only be given at the discretion of club owner.

By submitting this form, your child becomes a member of Gymnastics Australia (GA) and Gymnastics Queensland Inc and will be bound by all rules, regulations, policies and procedures of Gymnastics Australia and the State/Territory Association. This includes any rules, policies and regulations in relation to the GA Child Safe Policy.
